One in hundred children born with heart defect
Heart trouble and your child
Listen to your child’s heart for abnormality
Children with congenital heart defects can lead healthy life with surgery, Dr P Mankad
Chandigarh, June 23
When Baby Noor Fatima from Pakistan was operated in 2003 in India, the word of heart defects amongst newly born children spread around quickly. Fatima was amongst many cases that were fortunate enough to undergo surgery and get timely intervention for the ailment.
One, in every hundred children, is born with heart defects. In India there are more than 1 lakh kids being born with congenital defects every year. It remains commonest cause of death in first year of life, says Dr Pankaj Mankad, senior Paediatric Cardiac Surgeon from Edinburgh, Scotland, who will now be at Grecian Hospital, Mohali to serve children with heart defects once in two months.
Dr Mankad adds that in 99 percent of cases there is no cause known for this condition. While many children exhibit signs and symptoms of heart trouble right after the time of birth, others take couple of years to be detected as what we call “heart patients”.
Parents can act in a big way by identifying the symptoms of ailment in their child and help in early intervention. Parents should be aware of signs and symptoms such as baby not feeding well, getting tired or sweating while feeding because of the extra work needed to eat, having repeated chest infections, unexplained fever, not putting on enough weight or not growing properly.
Those children with mixing of blood appear dark or blue depending on skin pigmentation. This gives blue or purple hue to the skin. In some children, a buzzing sound is felt when hand is placed on the chest.
In 90% of kids a pediatrician is able to hear a murmur (abnormal heart sound) while examining the child with a stethoscope. An echocardiogram or ECG is done to help diagnose the defect
Older children with congestive heart failure are beyond time of rapid growth and therefore do not have major growth problems like infants. Their symptoms are usually related to their inability to tolerate exercise. They become short of breath more quickly compared to their peers and need to rest more often.
Shortness of breath can occur even with minimal exertion, such as climbing stairs or taking a walk if the heart failure is severe. In children with heart failure, passing out during exercise may be very serious and needs to be evaluated immediately. Appetite may be poor when heart failure is severe and weight loss or lack of weight gain can be seen even in older children.
Some children will retain fluid and will actually gain weight with heart failure and appear puffy. As it is harder to determine parameters for heart failure in older children, it is important to look for change in exercise capabilities or progression of symptoms with time.
Dr Shivpreet Singh Samra, Managing Director, Grecian Hospital, Mohali says, “Grecian Hospital is the only hospital beyond Delhi, capable of successfully conducting the child heart surgeries. With Dr Pankaj Mankad joining us, the children suffering from heart disease in this region have got a savior.”
Dr Pankaj Mankad is a senior heart surgeon who has been working in UK since 1983. He has been Consultant at the Edinburgh Royal Infirmary since 1990. He specializes in operating on complex heart diseases of children. He shall be operating at Grecian Hospital, Mohali every two months. He will be here till July 10 2009 and shall return in September.
Although a lot of facilities exist for heart surgery in adults, there is no dedicated facility for children’s hearts in North India. Patients have to go to Delhi. This is very expensive and inconvenient. The cost for such surgeries is about Re 2.5 lakhs in private hospitals. Institutes like AIIMS have very long waiting lists.
The children from poor and middle-income groups end up succumbing to the ailment. Over the past 3 years Grecian Hospital has carried out 74 surgeries on children with heart defects.
All those who wish to get their children treated by him should approach Grecian Hospital at 0 98146 10390/ 0 98550 70390. Depending on complexity of the case and financial situation of patient discounts can be offered as well.

Canada: Safest destination for migration
Canada remains the most sought after migration country amongst Indians
Mohali, June 18
Canada – the country of maple leaf – remains the safest and most sought after destination for Indians, especially Punjabi, with 3 million Indians settled there.
The only country in world to have Indians with a share in governance, Canada has a golden history that speaks for itself. In past 100 years of migration from India, the nation has never witnessed any racial abuse or attack.
“Canadian community largely comprises of Indians and even general bill boards have instructions written in Punjabi language. It’s a nation with zero tolerance towards racism and witnesses large number of Indians getting elected to various houses in state and cities,” says Lt Col BS Sandhu, President, Continental Institute for International Studies (CIIS), an institute that is an offshore campus of many Canadian universities.
Some of these universities include Thompson Rivers University, BC, Canada, Georgian College, Ontario, Canada, Red River College, Manitoba, Canada et al. It offers degree with option of shifting abroad to the parent campus. Also one can seek admission to various Canadian colleges through World Wide Education Services (WWES).
Total 51000 people have settled in Canada through World Wide immigration Consultancy Services including 39000 skilled worker professionals, 12000 businesspersons and around 9000 students. The nation is house to more than 2 lac Punjabi settlers with a strong share of Punjabis in governance too.
Liberal Party of Canada has 22 Indians, wherein one of them as served in two provinces as an MLA. Amongst Federals are Jag Bhaduria, Gurbax Singh Malhi, Herb Dhaliwal, Navdeep Bains, Ruby Dhalla, Sukh Dhaliwal, Ujjal Dosanjh. Gulzar Cheema is one of only a few Canadian politicians to sit in two provincial legislatures as member of Legislative assembly of Manitoba and Legislative assembly of British Columbia. Besides there are various members including Bharat Agnihotri from Ontario Vic Dhillon, Kuldip Kular, Harinder Takhar. The New Democratic Party (NDP) of Canada has nine Indians. Progressive Conservative Party of Canada (better known as Conservatives/Progressive Conservatives) have Deepak Oberoi, Gurmant Grewal, Nina Grewal, Harry Sohal, Raminder Gill elected to various houses.
“More than 25000 Indian students go to Canada on study permit every year. The country remains most sought after destination for higher studies all over the world with major and reputed universities offering courses of international standard. These include 6000 plus Punjabi students,” Sandhu says.
Canadian society is woven of multicultural threads reflecting everlasting harmony amongst native Canadians and Indians. Most immigrants prefer larger urban cities as it is easier to find jobs and its more accessible than other cities. The preferred cities remain Toronto, Ontario and Vancouver, B.C. These areas reflect multiculturalism as immigrants sponsor their other relatives here and they end up settling in the same place. And once cities start booming, other immigrants are attracted there adding to its multi dimensional society, Sandhu adds.
Sandhu says that it’s this multicultural harmony that beckons Punjabi students to head for greener pastures abroad. CIIS is perfect place for people aiming to touch horizon. There are under graduate programmes in engineering like Mechanical Engineering Technology Automotive Manufacturing-(META), Mechanical Technology Program (MTP), Aeronautical Engineering (B.Engg.AE). In computer studies there are options of Bachelor of Computing Science (BCS) and for business administration there’s option for Bachelor Of Business Administration (BBA).

Cardiac patients get new hope in cardiac resynchronization therapy (CRT)
New therapy set to bring smiles for cardiac patients
Chandigarh, June 7 ( ).Till six months ago 45-years-old Radhe Shyam, businessman from Karnal, was bed-ridden and dependent on others to take him for daily ablutions or even move around house.
Today he is an affluent businessman running his poultry business, goes to office, climbs up to 3 stairs and can easily walk for 2 to 3 km.
All this happened after he successfully underwent biventricular pacemaker implant, a cardiac resynchronization therapy (CRT) at Fortis Hospital under Dr HK Bali, MD, DM, Director, Cardiology Fortis Hospital and ex PGI cardiologist.
He adds that biventricular pacemaker is a type of implantable pacemaker designed to treat heart failure. This therapy becomes necessary when small chamber of Heart and two lower chambers of heart do not pump in unison. Such pacemaker helps these chambers to contract more efficiently. This pacing is called cardiac resynchronization therapy (CRT).
Biventricular pacemakers are relatively new. Previous pacemakers were used to pace only right side of heart. They were used when heart beat too slowly. By contrast, biventricular pacemakers pace the atrium and both right & left sides of heart.
They do not increase heart rate, but rather stimulate left and right chambers simultaneously. This helps left chamber pump blood more efficiently. These pacemakers do not increase heart rate unless needed when heart rate slows below a certain level.
Dr HK Bali says, “The CRT has offered a new hope of life for heart patients. Especially those, who have severe heart failure despite optimum medication and have certain echo and ECG criteria.
Quoting case of 70-years-old Kuldeep Kaur, a resident of Panchkula, Dr Bali says that she was admitted at Fortis hospital, Mohali three times in span of three months with severe heart failure. Twice she was required to be put on ventilator for treatment of lung congestion. Treating doctors diagnosed that she was suffering from symptoms of severe heart failure.
“Chambers of her heart were not synchronized. This reduced pumping efficiency of heart thereby reducing blood supply to various parts of body. Like Radhe Shyam, she too was treated with CRT,” says Dr Bali.
In Kuldeep’s case, biventricular pacemaker combined with an implantable cardioverter defibrillator (ICD) was used. This is for heart failure patients felt to be at high risk of sudden cardiac death due to some types of abnormal heart rhythms (ventricular tachycardia, ventricular fibrillation).
Unlike Kuldeep, Radhe Shyam had undergone angioplasty 10 years ago after suffering a heart attack. After symptom free seven years he again developed symptoms of worsening heart failure due to multiple asymptomatic small heart attacks.
Over next two years, he visited a number of leading hospitals in Delhi. “He was told that his heart had become extremely weak and there was not much hope for him. His symptoms continued to worsen and he became completely bed ridden,” tells Dr Bali.
He became dependent on others even for daily chores. Over span of few months he was admitted for 8 times in different hospitals for treatment. Each time he was given some treatment, discharged from hospital, only for the cycle to be repeated.
Six months ago, he was recommended CRT by having bi ventricular pacemaker implanted in his chest. Within fortnight of implant, his condition improved substantially. CRT has come as a boon and savior of people suffering from heart ailments.
Heart failure worse than cancer:
Dr Bali adds that heart failure is a problem worse than cancer. US analysis reveals that heart failure are estimated to afflict approx 5 million Americans and this number is expected to double within next five years. Also heart failure accounts for nearly 1 million hospitalizations annually, which are more than all forms of cancer combined.
“We Indians are at high risk population due to higher incidence of diabetes and hypertension. More patients survive short-term events and hence number of people who are at risk for developing heart failure subsequently is on rise,” says Dr Bali.
Another cause of worry for India is increasing life expectancy. Risk of heart failure increases dramatically with age. Heart failure affects approximately 10 of every 1,000 people over age of 65.
A word about CRT:
In CRT treatment an implantable device i.e.biventricular pacemaker is used to improve pumping efficiency of heart and increase blood flow to body. By improving blood flow, CRT may help to reduce heart failure symptoms and improve quality of life. CRT is intended to complement standard drug therapy, and dietary and lifestyle modifications.
The devices are implanted under skin in chest. Procedure is done under local anesthesia, so patient remains conscious. Device works silently, while patient can lead normal life.
Studies have shown that heart failure patients are at a higher risk for sudden cardiac arrest. Special CRT devices stop life threatening very fast heart rates by delivering mild electrical shock to patient.
As per study, patients with CRT implants had a 36% reduction in all-cause mortality, over 18 months, as compared with patients in control group.
These devices have come as boon for severely symptomatic patients of heart failure and in coming years their implantation is likely to increase markedly in our country, remarks Dr Bali.
